Electronic signatures are acceptable for ALL forms. Students can create a free account at: www.hellosign.com. The eSignature site will allow students to obtain an unlimited number of signatures for up to three documents per month/ 30 days. While there are other electronic signature programs students may utilize, we recommend this program because it is free for up to three documents and it is a secure site and process. If using hellosign.com please ensure signers are assigned in the correct order. For example, on the registration agreement, the first signer is the student, the second signer is the Thesis Chair, and the final signer is the department HIM coordinator or department chair.
Signed forms must be emailed to the Office of Honors Research at firstname.lastname@example.org or dropped off at Trevor Colbourn Hall 248.
Directed Readings Semester(s)
This must be submitted to the Office of Honors Research to be allowed to proceed to Thesis I semester.
Thesis Semester(s) (If you are graduating at the end of your thesis semester, please see below for an Intent to Graduate form)
These forms must be submitted to the Office of Honors Research during the final thesis semester to receive Honors distinction.
Thesis Attachment Form - This form verifies that students' theses have been submitted to an online plagiarism detection software (Turnitin or iThenticate) and do not contain plagiarism. Please note that it is the University of Central Florida’s policy that all students who are writing a thesis, regardless of major, must submit a copy of their thesis to Turnitin or iThenticate for review. No exceptions will be made with this requirement or this form. This form also provides options for the dissemination of students' theses. Students should speak with their Thesis Chairs about what publication option is best.
Thesis Approval Form - Following the successful completion of the thesis defense, this form must be signed by the student's entire thesis committee, the Department Chair or Honors in the Major Coordinator, and the Dean of the Burnett Honors College (this signature will be obtained by OHR). Students must submit a copy of this form with original signatures to the Office of Honors Research in the new Trevor Colbourn Hall, suite 248. Please note that all committee members who sign this form must match the committee that our office has on file.
